Acute appendicitis remains one of the most frequent reasons for emergency hospital admissions and common cause an 'acute abdomen' in young adults. The risk is 8.6% men 6.7% women throughout a lifetime, with frequency peaking second decade life. In 19% acute cases, there perforation, it has bimodal distribution. Appendiceal gangrene perforation appear to be caused by appendiceal lumen being blocked. Gangrenous free bacterial contamination peritoneal cavity result from ischaemic necrosis appendix wall. Extreme ages, diabetes mellitus, immunosuppressive diseases, obstruction are factors perforation.
